Please complete this Expression of Interest (EOI) form to help us understand your community’s capacity and support for planning the event. Ensure all sections are filled out, including designated individuals for key roles, to be considered for the upcoming year.
Applications submitted by a community must be completed in their entirety and must identify designated representatives from each of the following categories: Community Member, Healthcare Professional, and High School.
