Healthcare Heroes Award Nomination Form

At least three letters of support that describe the nominee’s notable commitment to the community and local service groups including involvement in innovative projects and/ or patient experience improvement or team-building initiatives, the obstacles the nominee has overcome to achieve their contributions, and any relevant anecdotes, narratives, or testimonials.

1. a community organization
2. a health professional in or near the community who can speak to the nominee's health-care background, experience, contributions, and scope of practice
3. another group or individual other than the nominator

4. Letters of support should include the following:

1. Details about your nominee’s notable commitment to the community and local service groups and how that commitment has improved your community
2. An outline of the obstacles the nominee has had to overcome to achieve their contributions
3. In the letter of support from the health professional in the community, particulars of his/her contributions to medical practice, education, and/or research
4. Any relevant personal anecdotes, testimonials, narratives, etc
