Nominations for the 2025 Rhapsody Healthcare Heroes Award are now closed.
Eligibility Criteria
For the Healthcare Heroes Award, the nominee can be an individual or team of health professionals, if they are an active member(s) of Alberta’s regulated health professions.
The rural Alberta community(s) in which the nominee (individual or team) lives and practises must meet the following eligibility criteria:
• It is a community(s) located outside the defined Census Metropolitan Area (CMA) boundaries of Calgary or Edmonton
• Has a population of fewer than 15,000 people – based on the most recent national census
• The communities in which the nominee(s) live and practice can be the same or neighbouring, as long as they are both defined as rural.
If the nominee is an Individual:
• Live and practise in rural Alberta for at least six consecutive years
• Have not received an individual Rhapsody Award in the last six years
If the nominee is a Team:
• Team members practise and the majority live within rural Alberta, or in a neighbouring community, to the one they serve
• Have not received a Rhapsody Healthcare Heroes Team Award within the last six years.
Nomination Requirements
To nominate a Team or Individual for the Healthcare Heroes Award, please provide the following:
- Name and contact information for the primary nominator o If the primary nominator is from outside of the community of the nominee, a secondary nominator from within the community must be included for the nomination to be eligible.
- Name, title, and contact information for the nominee
- A description of why the nominee is deserving of the award which could include but is not limited to; involvement in innovative projects, patient experience improvement, or team-building initiatives
- At least one high-resolution photo of the nominee(s) must be provided. Any other relevant media reports, pictures, etc. that help to support your nomination can also be provided. All media must be the highest resolution possible for social media sharing.
- At least three letters of support that describe the nominee’s notable commitment to the community and local service groups including involvement in innovative projects and/ or patient experience improvement or team-building initiatives, the obstacles the nominee has overcome to achieve their contributions, and any relevant anecdotes, narratives, or testimonials. These letters of support should demonstrate the following:
Healthcare support: a letter from a community organization as evidence of community support and impact, and how that commitment and involvement has improved the community, resident experience, and access to healthcare
Community Impact: a letter from a group or individual other than the nominator who can speak to the nominee’s community involvement and/or volunteer work outside of healthcare
Health Practise: a letter from a health professional or health administrator in or near the community who can speak to the nominee’s healthcare background, experience, contributions, and scope of practice
– how the nominee goes above and beyond to provide healthcare in their community
-how the nominee works within a multidisciplinary team environment to provide healthcare services in their community
-the involvement of the nominee in training students and colleagues (including other health professionals on the team)
-the attraction and retention of other health professionals to the community
-any programs, education initiatives, innovations, etc. that the nominee has championed or introduced to the community
Additional letters of support are welcomed but not required. Please limit the number of letters to a maximum of ten.
Please confirm the following as part of the nomination process:
- The nominee(s) agrees to be nominated and, by agreeing to be nominated and if the nomination is successful, the nominee(s) consents to participate in the recognition event, to appear in a video tribute celebrating the award recipient(s), and to the use of their image in RhPAP promotions
- The nominator/community is available and prepared to liaise with RhPAP on the award presentation event or integrate an award recognition component into an existing event.
