HQCA is now recruiting physicians to sign-up for their 2016-17 Primary Healthcare Panel Report to receive the report in early 2018.
Over the last year, the HQCA collaborated with patients; family physicians; PCN executive and medical directors; Alberta Health; Alberta Health Services; Alberta Medical Association; the College of Family Physicians of Alberta; the College of Physicians & Surgeons of Alberta; and, the Universities of Alberta and Calgary medical schools to revise the Primary Healthcare Panel Reports. This year’s report reflects the needs of primary healthcare stakeholders, and includes:
- Updated measures which are actionable and useful
- A new format for easier reading and interpretation
- A new dashboard to help identify successes and areas for improvement
These standardized physician patient panel reports use administrative health data to provide information about patient demographic characteristics, health status, and aspects of patient management and utilization patterns across the healthcare system. Additionally, they are a resource that physicians and their teams can use to strengthen the Patient’s Medical Home by providing reliable data and vital information that can be used to improve panel management, continuity, access, and quality of care for patients on a panel.
