Chronic Kidney Disease (CKD) is common affecting 12% of adults in Canada. In Alberta, primary care physicians care for approximately 95% of patients with CKD. Early identification of adults who are at risk and management can reduce the progression of CKD to kidney failure requiring dialysis or transplant as well as adverse cardiovascular outcomes.
Primary care physicians can now access several resources and point-of-care tools to help with CKD diagnosis, management and referral.
- Online CKD Clinical Pathway – an interactive tool providing guidance and support to physicians and allied health care professional in the identification, management and appropriate referral of patients with CKD. For more info (www.ckdpathway.ca)
- The Comprehensive Annual Care Plan (CACP) completed by primary care physicians facilitates complex disease management including CKD. For patients with CKD, the CACP and the Online CKD Clinical Pathway have been integrated within TELUS Health EMR Encounter Templates and Dashboard.
